Second opinions and travel-in orthobiologic care
My office is in Mount Pleasant, just outside Charleston, but a good share of the people I see drive in — from Hilton Head and Beaufort, from Columbia and Orangeburg, from Florence, Myrtle Beach and as far as Greenville. They come for one of two reasons: a straight second opinion on a shoulder or knee, or access to orthobiologic treatment that is not offered near them.
My partner and I have built this side of the practice deliberately. We do the surgery when surgery is the answer, and we spend just as much of the week helping people avoid it. That combination is still uncommon in the Southeast, and it is the reason a two-hour drive often makes sense.
When a second opinion is worth it
You were told surgery is the only option
Sometimes it is. Often there is a non-operative path worth trying first — physician-directed therapy, a targeted injection, or a biologic treatment — and it is worth knowing whether yours is one of those.
You were told nothing more can be done
“Live with it” is not a diagnosis. Chronic tendon and early arthritis problems in particular have real options that many general practices do not offer.
A previous surgery did not work
Revision shoulder and knee problems are a large part of my practice — failed rotator cuff repairs, recurrent instability, stiff or painful replacements.
You want to know if a biologic is legitimate for you
PRP, bone marrow concentrate, microfat and focused shockwave are genuinely useful for some problems and a waste of money for others. An honest answer requires an exam and your imaging.

How a travel-in visit usually works

Send your imaging and records ahead. X-rays, MRI discs or a portal link — reviewed before you arrive so the visit is spent on you, not on paperwork.

One thorough consultation. Full history, hands-on examination, in-office ultrasound when it helps, and a frank read of what your imaging actually shows.

Treatment the same day when possible. PRP is prepared in the office from your own blood; shockwave and ultrasound-guided injections are done at the visit.

A written plan you can take home. Including what your local therapist or physician should do next, so follow-up does not require another drive.
